School nursing is a specialized practice of nursing that advances the well-being, academic success, and lifelong achievement and health of students. Keeping children healthy, safe, in school, and ready to learn should be a top priority for both healthcare and educational systems. With approximately 50.1 million students in public elementary and secondary schools, educational institutions are excellent locations to promote health in children and the school nurse is uniquely positioned to meet the student's health needs.
A student’s health is directly related to his or her ability to learn. Children with unmet health needs have a difficult time engaging in the educational process. The school nurse supports student success by providing health care through assessment, intervention, and follow-up for all children within the school setting. The school nurse addresses the physical, mental, emotional, and social health needs of students and supports their achievement in the learning process.

Asthma Care Plan
An Asthma Care Plan, as provided by the American Lung Association, is a personalized medical document that outlines how to manage a student’s asthma while at school. Also known as an Asthma Action Plan, it is developed by a healthcare provider and shared with school staff to ensure consistent, informed care throughout the school day. The purpose of the Asthma Care Plan is to help students manage their asthma proactively, reduce the risk of asthma attacks, and ensure they can participate safely in all school activities. It also empowers school staff with the knowledge and tools needed to respond quickly and appropriately in case of an asthma emergency. -
Diabetic Long Form
The Diabetes Medical Management Plan (DMMP) Long Form, as provided by the American Diabetes Association (ADA), serves as a comprehensive, detailed guide for managing a student’s diabetes care while at school. Completed by the student’s healthcare provider, the DMMP outlines individualized medical instructions to ensure that school personnel can support the student’s health, safety, and academic participation throughout the school day and during extracurricular activities. -
Diabetic Short Form
The Diabetic Management Plan Short Form is a simplified medical document that outlines the essential care and treatment instructions for a student with diabetes while at school. It is required to be completed by the student’s healthcare provider and shared with the school nurse and staff to ensure safe day-to-day management of the student’s condition. -
Food Allergy & Anaphylaxis Emergency Care Plan
An Allergy Action Plan is a medical document designed to help schools and caregivers respond quickly and effectively if a student experiences an allergic reaction. Completed by a licensed healthcare provider and signed by the parent or guardian, the plan outlines the student’s specific allergies, symptoms of a reaction, and clear, step-by-step instructions for treatment, including the use of emergency medications like epinephrine (EpiPen) or antihistamines. The purpose of the plan is to ensure that school staff and other caregivers can respond immediately and appropriately in the event of an allergic reaction, minimizing risk and protecting the student’s health and safety. It is a critical part of a coordinated care approach for students with life-threatening allergies. -
Food Allergy/Disability Substitution Form
The Food Allergy/Disability Substitution Form is a medical document used by schools to accommodate students with food allergies or other dietary disabilities that require meal modifications. This form must be completed and signed by a licensed medical professional, such as a physician, nurse practitioner, or physician assistant. The purpose of the form is to ensure that schools, particularly child nutrition departments, provide safe and appropriate meals for students with medically diagnosed dietary needs in compliance with federal laws, such as the Individuals with Disabilities Education Act (IDEA) and Section 504 of the Rehabilitation Act. Once submitted, the school nutrition staff uses the information to modify meals accordingly and help protect the student’s health and well-being during meal times. -
Immunization Letter
This letter serves as a reminder to get your child vaccinated early before the busy back-to-school rush. State law requires all students in Texas pre-kindergarten and childcare facilities to be immunized against certain vaccine-preventable diseases to protect the health of both the children and the community. Medication Administration Form
This is a consent form required by Hallsville ISD for administering medication, per HISD school board policy. -
Seizure Protocol
This form is designed to help create a plan for managing student seizures. It consists of questions about seizure history, medications, precautions, and other considerations. This form should be completed jointly by the student's parents and treating physician, and provided to the campus nurse or other appropriately identified personnel. (Texas Education Agency) -
SHAC Application 2024-2025
A School Health Advisory Council (SHAC) is a group of people selected by the school district from different areas of the community and from within the school district. This group assists the district in ensuring that local community values are reflected in health education instruction.
